Does the gate appear to be opened? Closed? What can we guess closing it or opening would do to the water level?

It's very much closed. Opening it would lower the water level significantly, the entire gate is wide enough to sail a modestly-sized ship through, and it seems like a goodly portion of it is underwater.

Absolutely none of either, unfortunately. The gate looks resolutely closed. Looking closer at it, are there... claw marks? Just faintly, in various patches, especially near the edge where the gate meets the wall.

Climbing over the large gate, sticking close to the cold metal walls, you arrive safely on the other side. There's much the same to be seen, except for a thin crack in the wall that is letting in a tiny dribble of fluid. The wall to either side of the crack is stricken with even more rust than usual. You're now on the NE side of the lake.1d6

Is the crack near the gate (ie. would we guess the gate is holding back some reservoir)?

To clarify, the water gate is more like a large circular plug, flush with the metal walls on either side. There's no way over it unless the gate is open, but you can get past it and keep going around the lake (which you have already done). The crack in the wall with water coming through it is higher up than the lake, quite a lot higher in fact.

If water is coming into us from a crack in the wall up above then pulling the plug so to speak will probably flood us. Let’s keep heading around the lake. We could open it by shrinking the plug.

The crack is much larger than the trickle of water coming through it, it's not under any kind of pressure. Coming further around the lake, the path starts to get steeper and more jagged. It's not quite the vertical cliff that was blocking the other way, but it'll still be a bit of a climb, with a drop into the viscous sludge below on the other side.

Scrambling up the cliff goes much easier once Blerble is at the top securing the rope with Terrance, and you aren't troubled by anything. At the top of the cliff there is a narrow circular tunnel leading north. You can see a number of weird-looking hatches spaced evenly along the floor of the tunnel, like lump manhole covers.1d6

It's metal. It sounds like there is a hollow space on the other side. They shift a little when you tap them, but you can see that the handle on this side has a latch. It can be easily opened from this side, but would be very difficult from the other.

Terrance huffs and puffs (not that he has lungs) and strains. He still hasn't quite gotten over that time with the swinging axes, but eventually he manages to pull the hatch up. When nothing happens, you look down and see: wXJKQ5jt1d6
A... tiny hospital? There's a matronly looking woman staring up at you, a tiny bed, a soft white light coming from somewhere you can't quite see, bandages and blankets and cough syrup and any number of other amenities in the tiny drawers making up the other side of the room.1d6

What size are these things? How tall is the woman? I will whisper to her “hello. Who are you? What are you doing?” Does there appear to be a tiny door in her little tiny room or is it a self contained thing?

All the items in the room seem to be about normal size, but very closely packed, while the furniture is small and bends to match the shape of the cylindrical room. She doesn't respond to anything you say, except to smile a little more. There's no tiny door, the only way in or out would be this hatch you've just opened.
Her eyes are glittering with warmth. She does not seem alright.

It's a fair way down to reach even the highest drawer. The cylindrical room is more like a pit than anything else.

The soot sprites boil out of their container and drift down into the room. Several of them just keep falling, landing on surfaces and leaving nothing but a dead smudge of ash. Most of them waft as quickly as they can back to you, chittering in fear and alarm, but many are left behind, rising as far as they can before falling again, to drift into nothingness.
The nurses's eyes keep twinkling with warmth. Did one of those dead soot sprites land directly in her eye?
