I am heading for the worm queen. Which is west, take the minecarts, east, down to tall room, then climb the grappling hook to the balcony "WELCOME BACK MY DEAR CHILD. ARE YOU WELL? WHAT TIDINGS HAVE YOU BROUGHT ME FROM THE BUSY WORLD?"((Also I just checked and you asked the Worm Queen to extract the spells from Andsaca's skull. Which spells were they again? And more importantly, which would you want first?))

From the shadows behind the throne shambles the corpse of a dwarf. It's full of bloodless holes and missing an eye, and from every socket and pore squirm countless finger-sized worms. The corpse is being pushed and pulled by larger siblings, but none as big as you. The assemblage is crooked and shaky and falling apart, but the corpse-nest reaches out a hand and takes the weird wand. More worms sprout out of the dead dwarf's mouth, and pull the wand down it's throat slowly.
"THEY ARE YOUNG AND SHY, BUT STILL LEARNING. THIS TOME OF YOURS MAY TAKE SOME CAREFUL CONSIDERATION AND DECONSTRUCTION, AND I THANK YOU FOR IT. HERE, A GIFT FROM YOUR FATHER IN RETURN."
A patch of floor tiles creak upwards, pushed by squat worms, revealing a little shrine to Andsaca's skull. It is frothing with pulsing and twisting worms, several with large sacks of some fluid taking up most of their body. One of these coloured worms squidges it's way out of Andsaca's eye-socket, and shuffles towards you.
"THE KNOCK SPELL, GIVEN FREEDOM AND FORM. THIS ONE IS A LITTLE... UNSTABLE, MORE SPELL THAN FLESH. YOU MIGHT CONSUME IT IF YOU WISHED TO KNOW THE POWER YOURSELF, BUT YOU ARE TRULY PRECIOUS TO ME MY SON, AND I AM NOT CERTAIN THAT THE PROCESS WOULD LEAVE YOU UNHARMED."
((Basically you can eat the Scroll-Worm and learn Cantankerous Knock, but it might harm you or cause mutations or other such things. Or, you can let the Scroll-Worm cast spells, a little like an unstable wand. Or you could try and convert it into a more portable form))

Hmmm I will accept it graciously. Gonna spend some time chit chatting. If anything important comes up let me know otherwise I am probably heading out. Probably down to the tall room then north to goblin smelling room then down the metal ladder past the stone that lets you talk to diseases, south to the rotating blade trap (currently off) then gonna look around at some of the stuff left here.

Ah well at least I know which floor it is on then. (Floor +2 right?) I should make sure I have some of those assorted gems on me (let me know if I should backtrack to cell block zero to get them) otherwise I would like to ask where the collapsed passage connected to this room leads.

Floor +1, in fact (even though you are up a balcony, it still isn't anywhere close to going up a full Floor).
You can have assorted gems on yourself, sure.

Down the grapple hook, then down the interminable ladder leading from floor +1 to floor 0. As a worm I presume you'd squirm yourself around each rung on the way down, but the rush of blood (do worms have blood?) to your head still makes it uncomfortable going.
At the base of the ladder, the hallway splits to the north (where you can hear the sound of the river burbling by) and the the south-west. The large circular blade-trap is indeed still, and the shattered lights means you have only your torch to see by. There are four ledge exits, NW (where you are right now), SW (the small looted chamber), SE (to the strange altar) and NE (unexplored).
Crawling carefully around the blade, you check out the NW exit. It splits into two almost immediately, north and south. You can hear a strange sloshing sound that doesn't sound quite the same as the river coming from the north.

The ladder enters from the NW corner ((everything has been rotated 45 degrees anti-clockwise, I can't for the life of me remember why but it does fit the corner of the map better))
Checking north, you come to a large circular room. Half of it is made up of a large and very slightly stagnant pool of water. It's a little murky, but you can see something under the surface. Something moving, and quite large. What do you do?

I will have the soot sprites make a small fish looking coal object and I will use Terrance to toss it towards the middle of the pool.

It floats at the top of the water, rapidly getting soggy. There's a swish, and it's gone. There's a swirling in the water, and a large rubbery tendril rises to the surface. It's holding the wet fish-thing, and places it gently back on the edge of the pool of water. There's a tiny nibbled chunk taken out of it. Another rubbery tendril rises to the surface, it's holding at the very tip, ever so delicately, a shimmering lumpy pearl.
What do you do?

I will have Terrance go and grab the pearl delicately.
A flying hand is pretty hard to grab especially one that can shapeshift and not disturb the water.
If that goes well I hope to talk to the creature from as far away as possible in the room.

The pearl is curled up into the tendril as Terrance approaches. What you can see of the creature looks pretty emaciated, to be perfectly honest, and an intelligent creature would have to be quite desperate to take even a small bite of a fake charcoal fish.

Everyone seems to be starving in this dungeon. *sigh* “I will be back great one.” I want to go back up the ladder to the tall room, north to the goblin smelling room, west to the minecart and south a bit to the spiral lichen area. There should be some blood turnips there to harvest.

No problems, you've got this place pretty much under control. A bit of hiking and climbing later, the creature swaps the shiny pearl for a turnip, which is immediately torn to pieces and devoured. The water is pinkish for a few moments, and then clears.

The pearl just seems to be a large, natural pearl. The tendril waves goodbye as you leave. Perhaps... a little sadly? Another tendril is holding a second pearl.
Heading south, your nose (if you have one?) wrinkles at a foul stench. Peering into the room, you can see a set of steps in arches across the room, scattered with puddles of blackish-green fluid. There is an exit to the south, you can hear an echo of crashing metal and gruff cursing coming from somewhere much further to the south.

Down the steps, wriggling carefully around the goop, and through the chamber leads you to another square room. There are two large slabs (Metal? Marble?) of material arranged in parallel, as high as a man. You can see that the interior of each is finely carved with words or runes (but I'll assume you don't stare too hard, just in case).
You can see another exit in the south-west corner of the room, you can easily walk around the slabs to get to it if you wish to continue south.

They are very complex, with layers of runes and writing on the inner edge of both slabs. The outside is plain, and scuffed in a few places, while the interior between the two slabs is perfectly smooth. There is no dust at all on the floor between them either.
